If you do not see your insurance policy above or have a question about whether you are covered at Vision Care Associates, please contact one of our many convenient locations.
Payment is expected at the time of service for all procedures unless prior arrangements are made. Services covered by an insurance company with which we have a participating agreement will be billed to the insurance company first.
Vision Care Associates will submit to most private insurance plans. Vision insurance benefits cannot be combined with any of our other promotional offers, coupons or discounts. If your insurance requires a co-payment or has non covered services, these will be due at the time of service.
Flexible Spending Account
Do you participate in a Flexible Spending Account (FSA) through your employer? Your flexible spending account dollars can be used for prescription eyewear, including prescription sunglasses and contacts at any of our locations.
If billing is necessary, a statement will be mailed to you that is due within 30 days. Charges and payments for services received during the last few days before your billing date may appear on the following month’s statement.
For your convenience, we accept VISA, MasterCard, Discover, American Express and bank debit cards.
Please bring all health insurance information with you. We will need a copy of any insurance cards for our records. If you have any questions regarding Medicare, insurance policies and/or procedures, do not hesitate to ask. We will do our best to assist you.
